A conversion certificate hand-written on the letterhead of The United Hebrew "Beth Din" Office, with [Rabbi] K. Abraham Goldberg's name and address, in Hebrew and in English, printed at the top of the page.
The certificate was issued on Tuesday, the 15h day of Tammuz 5699 (July 2, 1939) to a woman named Sarah bat Avraham Avinu. It certifies that she has accepted upon herself the mitzvoth and that she immersed herself in front of the witnesses …that she knows how to conduct herself… to observe and to do the mitzvoth of Hashem...with observing the Sabbath, with kosher foods, learning the blessings….
It does not have the signatures of the witnesses, nor does it give her original name by which to identify her.
