Two blank receipt forms from the Talmud Torah Yeshivath Haye Olam & Orphans Kitchen of Jerusalem. One is printed on white paper, with blue ink and a greenish background on the receipt form itself, and the other one is printed on green/blue paper with black ink. The area of the receipt itself has the same green background as the other one.
The forms have text in Hebrew, Yiddish and English. On the back of the forms is a prayer to be recited before candle lighting on Friday night (in Yiddish) and also "Statutes Abridgely" [sic] in English and Yiddish. These statutes give the rights and privileges of the contributors to this charity. Prayers will be offered by the students and "learned men"of the Yeshiva four times a year at the "Holy Wall" and at the tomb of "Our Mother Rachel". In addition, a Prayer of Blessing will be offered every Shabbat and Festival.
For those who contribute $5 per year, after his death a "learned man" will pray in his memory. For $20, plus $5 per year, in addition to praying for him, a "learned man" will study a chapter of Mishnayos and pray "Kadish" during the first year after his "departure." For $50, ten "pious Jews" will pray for his memory during the first month after his death, and a chapter of Mishnayos will be studied in his memory on his Yahrzeit.
